IN (5, NULL) expands to manager_id = 5 OR manager_id = NULL. The comparison manager_id = NULL is UNKNOWN for every row - even for Alice and Carol whose manager_id is NULL - because equality with NULL is never TRUE. Only Bob matches manager_id = 5. To find NULL values you must use IS NULL explicitly.
